![]() Diverticular disease of the colon. Many health- conscious men can recite their cholesterol counts, blood pressure readings, and PSA levels without even glancing at their medical records. But few of these well- informed gents can tell you if they have diverticular disease of the colon, even though it's an extremely common condition. That's understandable, since the most prevalent form of the problem, diverticulosis, produces few if any symptoms. Still, when complications develop, blissful ignorance about diverticulosis abruptly gives way to an unwelcome education about the pain of diverticulitis or the bleeding of diverticulosis. It's a learning experience that's particularly unfortunate, since diverticular disease is largely preventable. Your colon. The colon is a 4 1/2- foot- long tube that constitutes the final portion of the intestinal tract. The food you eat is mostly digested in the stomach and small intestine. Residual material enters the colon, or large intestine, in the cecum, which lies in the right lower portion of the abdomen (see Figure 1). From there, digested material travels up the ascending colon, across the transverse colon, and down the descending colon to the final portion, the sigmoid colon, in the lower left part of the abdomen. The intestinal contents take about 1. When healthy, the colon is a smooth cylinder lined by a layer of epithelial cells. The bacteria that are packed into feces by the hundreds of millions are responsible for the inflammation, but doctors don't fully understand why some diverticula become infected and inflamed while many do not. A current theory holds that the wall of the diverticular sac becomes eroded by pressure, trapped fecal material, or both. If the damage is severe enough, a tiny perforation develops in the wall of the sac, allowing bacteria to infect the surrounding tissues. In most cases, the body's immune system is able to contain the infection, confining it to a small area on the outside of the colon. In other cases, though, the infection enlarges to become a larger abscess, or it extends to the entire lining of the abdomen, a critical complication called peritonitis. Pain is the major symptom. Because diverticulosis typically occurs in the sigmoid colon, the pain is usually most pronounced in the lower left part of the abdomen, but other areas may be involved. Fever is also very common, sometimes accompanied by chills. If the inflamed sigmoid is up against the bladder, a man may develop enough urinary urgency, frequency, and discomfort to mimic prostatitis or a bladder infection. Other symptoms may include nausea, loss of appetite, and fatigue. Some patients have constipation, others diarrhea. Diverticulitis: Diagnosis. A physician's exam may reveal tenderness over the inflamed tissues, typically in the lower left abdomen; less often, the doctor may feel swelling. As in other infections, the white blood cell counts are usually elevated. But because these findings are non- specific, further testing is required to establish the diagnosis. The best test is a CT scan of the abdomen, ideally performed after the patient receives contrast material both by mouth and intravenously. And a month or two later, after treatment has quieted things down, the patient should have a colonoscopy, both to evaluate the diverticular disease and to be sure that no other abnormalities are lurking. Diverticulitis: Therapy. Since bacteria are responsible for the inflammation, antibiotics are the cornerstone of treatment. And because the colon harbors so many bacterial species, doctors must prescribe treatment that will target a broad range of bacteria, including Bacteroides and other anaerobic bacteria that grow best without oxygen, as well as E. A common approach is to prescribe metronidazole (Flagyl, generic) for the anaerobes along with ciprofloxacin (Cipro, generic) or trimethoprim- sulfamethoxazole (Bactrim, generic) for the aerobes. Amoxicillin–clavulanic acid (Augmentin) is effective against both types of bacteria and is a good alternative. Needless to say, there are many variations on the theme, and doctors must always take their patients' allergies and general health into consideration when they prescribe antibiotics. Patients with mild- to- moderate diverticulitis can take their antibiotics in pill form at home, but patients with severe inflammation or complications (see below) should receive intravenous (IV) antibiotics in the hospital, and then finish up with pills at home. For home treatment, that means sticking to a diet of clear liquids for a few days, then gradually adding soft solids and moving to a more normal diet over a week or two. Patients with abscesses tend to be sicker than those with uncomplicated diverticulitis, and they have higher temperatures, more pain, and higher white blood cell counts. If a story hews to some very familiar pattern, it feels stale. But if absolutely anything can happen, a story becomes as unengaging as someone else’s dream. The gnarly zone is lies at the interface between logic and fantasy. I see my tales as simulated worlds in which the characters and tropes and social situations bounce off each other like eddies in a turbulent wake, like gliders in a cellular automaton graphic, like vines twisting around each other in a jungle. When I write, I like to be surprised.(4) My early mentor Robert Sheckley was a supremely witty writer. Over the years I got to spend a few golden hours in Sheckley’s presence. And I think it’s safe to say that wit, rather than mere humor, was his primary goal. I had just gotten the bad news from my chairman at lunch time, and I’d spent the afternoon going through my second to last spore of geezel. Targeted Keto Dieting—The final variation of keto dieting, TKD, utilizes intermittent periods of carbohydrate intake specifically around the workout timeframe. The goal here is to provide enough glucose to enhance athletic performance without inhibiting ketosis for extended periods of time. It is generally recommended to start with a “break in” period of SKD for several weeks and see how your performance and overall energy levels feel. From there, you should have a better idea of which diet to utilize for longer- term purposes. You may also be wondering which variation is best for fat loss and which is best for muscle building. Well, assuming your calorie intake is where it should be to reach your goal, the variation of keto diet you use won’t play too of a factor in your progress. Some people may make the argument that CKD and TKD are more suited to individuals looking to build muscle since carbohydrates are protein sparing and insulinogenic, while SKD is best for fat loss since insulin output will be relatively low. However, these short- term fluctuations in insulin secretion won’t play as much of a factor as total calorie intake will in the long run. Below is a short synopsis of what you should consider when picking which variation of keto dieting to use: SKD—This is best suited to individuals who are generally sedentary and find that their workout performance is not hindered when carbohydrates have been greatly restricted. This is also a good option for people who don’t train very intensely and/or are highly insulin resistant. TKD—If you’re someone who trains intensely several times per week and finds that your performance is worsened by chronic low- carbohydrate dieting, it is wise to consider implementing short carbohydrate feedings before and possibly after training. This is also a good option for people who are advanced trainees who can benefit from the carbohydrate intake but don’t do well with large carbohydrate re- feeds. CKD—This is generally considered the “most advanced” form of keto dieting since it will take some trial an error on the users part to optimize the time between their carbohydrate re- feeds and the amount of carbohydrates they eat on re- feed days. If you’re an advanced trainee who exercises intensely throughout the week and still finds performance to be lacking on SKD and TKD, then consider trying CKD and seeing if that helps bring your performance back up to speed. This will give you your baseline SKD nutrient intake. CKD and TKD users will still use their SKD calculations but there are a few adjustments to be made that I will touch on below. The general rule of thumb for people who are looking to lose fat is to aim for roughly a 5. However, this is a very raw generalization and may differ for some individuals depending on their physiological tendencies and other factors. So in total, this person’s nutrient breakdown is: 1. Some people will insist that no direct starch or sugar sources should be ingested as they will prevent ketosis but in minute amounts that is highly unlikely, especially for larger individuals. That being said, during the SKD portion of one’s keto diet, due to the inherently low carbohydrate intake and high fat intake you’ll likely find that these foods are good options. Animal proteins (especially red meats). Eggs (whole and white only). Full- fat dairy products like cheese, cream, butter etc. This method will take a lot of personal experimentation, as you will have to gauge how many carbohydrates you ingest and how you feel in the days thereafter. The main thing to consider when starting a carbohydrate re- feed is to limit fat intake; do not continue eating large amounts of fats while loading up on carbohydrates. However, protein intake should remain the same (or maybe even go up a touch for the calorie content). Using the previously mentioned 1. SKD macronutrient breakdown, we’ll suggest some baseline recommendations on how they could setup CKD with a once- weekly carbohydrate re- feed based on insulin sensitivity/tolerance for carbohydrates. Set protein intake at 1g/lb of lean body mass: 1. Low insulin sensitivity—set carbohydrate intake between 1- 1. Moderate insulin sensitivity—set carbohydrate intake between 2- 2. High insulin sensitivity—set carbohydrate intake between 3- 3. Now, as before, just simply calculate the leftover calories and divide by 9 to figure out how many grams of fat you should eat on your carbohydrate re- feeds*NOTE: If you’re on a cutting diet and moderately or highly insulin sensitive, raise your calorie intake back to maintenance levels (or maybe a even slight surplus) on carbohydrate re- feed days. Sunday (carbohydrate re- feed day)—1. Alterations for TKDLet’s take a look at how to implement carbohydrates in a targeted fashion on a keto diet (e. Some of the main complaints people have about ketogenic diets are that while they are undeniably effective, the lack of carb powered energy can make it hard for them to work out at their normal level, and that the absence of carbs can make the diet very hard to stick to long term – people simply enjoy eating carbs. If you find a ketogenic diet difficult for either of these reasons, a cyclical ketogenic diet could be the answer. What is a Cyclical Ketogenic Diet? A cyclical ketogenic diet is where you follow a standard ketogenic diet from Monday to Friday, incorporating three workouts into your week, and then on weekends you load up on carbs and don’t exercise. This works because carb loading allows you to store up some carbohydrate energy for the week ahead, helping you keep a reasonable level of strength up for your workouts (you won’t break any personal records, but you’ll feel good) and preventing your body from going into “starvation mode” (where weight loss slows down) as a result of the low calorie intake on your ketogenic diet days. It’s possible to measure it by testing urine, blood or breath samples. But there are also telltale symptoms, that require no testing: Dry mouth and increased thirst. Unless you drink enough and get enough electrolytes, like salt, you may feel a dry mouth. Try a cup of bouillon or two daily, plus as much water as you need. Increased urination – another ketone body, acetoacetate, can end up in the urine. This makes it possible to test for ketosis using urine strips. It also – at least when starting out – can result in having to go to the bathroom more often. This is the main cause of the increased thirst (above). Keto breath – this is due to a ketone body called acetone escaping via our breath. It can make a person’s breath smell “fruity”, or similar to nail polish remover. This smell can sometimes also be felt from sweat, when working out. Increased energy – perhaps after a few days of feeling tired (the “keto flu“) many people experience a clear increase in energy levels. This can also be experienced as clear thinking, a lack of “brain fog” or even as a sense of euphoria. How Do You Measure Ketosis? There are three ways to measure for ketones, which all come with pros and cons: Urine strips. Breath ketone analyzers. Blood ketone meter. Urine strips. Urine strips is the most simple and cheap way to measure ketosis. It is the first option for most beginners. You dip the stick in your urine, and 1. If you get a high reading (a dark purple color) you’ll know that you’re in ketosis. Pro: Ketone strips are available in regular pharmacies or via Amazon and they’re very cheap. A strongly positive test reliably proves that you’re in ketosis. Con: Results can vary depending on how much fluid you drink. The strips don’t show a precise ketone level. Finally and most importantly, as you become increasingly keto- adapted and your body reabsorbs ketones from the urine, urine strips may become unreliable even if you’re in ketosis.
